Keep your information up-to-date with BNF to avoid loss or suspension of coverage for you and/or your dependents. The COB form must be completed each time you are adding a new dependent (spouse or child). It is recommended that each member complete the COB at least once a year to ensure accuracy and efficiency when it comes to benefits.
